    PROGRAM CONSENT FORM
    By registering online, I acknowledge that I understand that my child will be participating in the Western Springs Blazers/Wildcats Traveling Basketball Program. I give permission for my child to participate as outlined below. *I also acknowledge that additional fees may be owed for my child's team at the completion of the season.

    MEDICAL INFORMATION/CONSENT
    Safety is a key concern in any athletic endeavor. Your child will be participating in many basketball practices and games during the next few months. PLEASE BE AWARE THAT THERE IS INHERENT RISK OF INJURY PLAYING BASKETBALL. If you believe this activity could endanger your child, please do not sign up or drop him/her from this program. Also, please make sure your child has had a physical checkup from his/her doctor and is physically able to participate in this strenuous activity.

    Finally, due to the nature of a traveling team, the players will be playing games in other communities. If an EMERGENCY SITUATION arises involving your child, we will take quick action. The coach will make every effort to contact a child's parents, but failing to reach you, he/she will exercise his/her best judgement including making a decision to contact 911 services for assistance. The MEDICAL INFORMATION and CONSENT you provide will be with the coach during the season. If there are any questions or concerns about these items, please contact Mike Kenny at 708-246-9070 x428.

    GENERAL ACTIVITY/COVID WAIVER
    Please read this carefully and be aware that by registering for and participating in programs or by registering your minor child/ward for participation in programs you will be waiving your rights and/or the right of your minor child/ward to all claims for injuries you or your minor child/ ward might sustain arising out of programs and you will be required to indemnify, hold harmless and defend the Village of Western Springs for any claims arising out of participation in Recreation Department programs.
    RISK OF INJURY: "As a participant in the programs, or as a parent or legal guardian of a participant under 18 years of age, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of injuries, including death, damages, or loss which I may sustain as a result of participating in any and all activities associated with Recreation Department programs."
    WAIVER OF INJURY CLAIMS: "I agree to waive and relinquish any and all claims I may have arising out of, connected with, or in any way associated with the activities of the programs."
    RELEASE FROM LIABILITY: "I do hereby fully release and discharge the Village of Western Springs and its officers, agents, and employees from any and all claims from injuries, including death, damage or loss which I or my minor child/ward may have or which may occur on account of participation in the programs."
    INDEMNIFY AND DEFENSE: "I further agree to indemnify, hold harmless and defend the Village of Western Springs and its officers, agents, and employees from any and all claims from injuries, including death, damages, and losses sustained by me or my minor child/ward and arising out of, connected with, or in any way associated with the activities of the programs. In the event of any emergency, I authorize the public entity to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed reasonable and necessary for my minor child’s immediate care and agree that I will be responsible for payment of any and all medical services rendered."
    I have read and fully understand and agree to the above stated conditions of participation.

    COVID-19 MONITORING PERMISSION
    In consideration of myself and/or my child/ward being allowed to participate in the Activity, I grant the Village of Western Springs, its officials, agents or employees full permission to monitor for symptoms of COVID-19, which may include being asked a series of questions:
    --Have you had a cough or a fever?
    --Have you been in contact with anyone who recently traveled abroad, or anyone showing symptoms for COVID-19?
    I understand and agree that if I and/or my child/ward has a temperature, shows symptoms of COVID-19 or answers any questions in the affirmative, I and/or my child/ward shall not be allowed to participate in the Activity.
    I agree that I and/or my child/ward will perform a temperature scan prior to attending any Activity and will not attend the Activity if temperature is above 100.4º F. I agree not to attend the Activity if I and/or my child/ward show any symptoms of COVID-19.

    I agree to contact the Village of Western Springs Recreation Department if I and/or my child/ward tests positive for COVID-19 and have attended the Activity. (Your personal information will not be shared).
    Extension of Participant Liability Waiver and Hold Harmless Agreement. I hereby agree and understand that the terms and provisions of the Participant Liability Waiver and Hold Harmless Agreement contained on the Registration Form is extended to include COVID-19 exposure, infection or the monitoring of COVID-19 exposure as provided above.
    I have read and fully understand and agree to the above stated conditions of participation regarding Covid-19.
    Western Springs Recreation Dept and Christopher Spartz Basketball:
    Blazers and Wildcats Travel Basketball Bubble

    The WINTER program will be broken down into 2 Seasons.

    SEASON 1: November 1-January 1
    2 practices per week (14 total): all held in our Bubble at WS Rec Center; 1hr 15min practices: 2/3 CSB Skills Development Coach Run; 1/3 Coach Run

    12 potential inter squad game dates on Saturdays and Sundays: all games held in our Bubble at WS Rec Center; all games coached by coaches.

    Each coach will advise when their particular teams have been set.

    SEASON 2:* January 1-last week of February. Season 2 will be predicated on protocol updates from our State and Governor.
    *Players must commit to the first season to be a part of the second season.

    With the objectives listed above, the Rec Dept and CSB Skill Development Coaches will work hand in hand with each coach to roll out a plan of attack that can be carried out and executed through January 1. Season 1 will give the CSB Skill Development Coaches a real chance to target areas that each team needs to grow and chart their growth.

    Based on the rules and regulations handed down by the Governor, we will only be allowed to compete in inter squad games, which means we have to align our practice times with the teams they will be scrimmaging against. We will know what that looks like when we have our teams in place.

    SENIOR CENTER MEMBERSHIP
    Membership, which is open to the general public, provides participation in our community of activities and events geared specifically to the interests of the older adult. Membership also supports the need of seniors to socialize and make new friends. We are located at the Grand Avenue Community Center, 4211 Grand Avenue, Western Springs, 2nd floor in the Neal Room (elevator access available).     GOLD level membership is $50/Residents, $60/Nonresidents and includes all activities including unlimited exercise and unlimited classes.

    SILVER level membership is $40/Residents, $50/Nonresidents and includes all activities except exercise and classes.

    BRONZE level membership is $20/Residents, $30/Nonresidents and includes library book borrowing, movie borrowing, movie watching, and regular bridge subs.

    Spouses are ½ price for each level.

    Join us for a baking class from home! Each week we will pair a delicious recipe with an imaginative story. We will work through the recipe together in our Zoom class, and while we wait for our creation to bake we will read our story, then expand on the themes with drawing and sharing time.     Illinois Shotokan Karate Club is Chicagoland’s premier karate school taught under the instruction of John DiPasquale, a four-time National Champion, U.S. National Coach and President of the U.S. National Karate Federation.

    PRE-KARATE (4-7 years)
    Young children develop flexibility, strength, and coordination in fun, yet disciplined activities. This program is specifically designed for growing minds and bodies to enhance memory and agility. Watch your child’s confidence blossom and their skills improve.

    PARENT/CHILD (7 years and up)
    Parent/Child classes are a great way to spend quality time with your child, while conditioning muscles, developing coordination and improving cardiovascular fitness. Both parent and child must each sign up for class separately.

    YOUTH/ADULT (8 years and up)
    These classes will put students in touch with their bodies and offers a good mix of strength training, with cardiovascular and flexibility exercises. Students will increase their coordination, agility and poise, as well as learn lasting personal safety skills. They also benefit from the mental stimulation of learning the choreographed moves of the kata and the discipline of conforming to the class rules.

    - Uniforms and Belt-Testing are options available through the ISKC instructor.
    - Please note that the Youth/Adult and Parent/Child classes may train together.
    - Beginner students will not be permitted to register into the current session after the second class has met. Registration for continuers through advanced students will be accepted after the start date, but the full class fee is required.

    LIL HOOPERS
    Let the first time your son or daughter pick up a basketball be with Christopher Spartz Basketball’s Lil Hoopers! This is a great way to get acclimated with the premier basketball program of Western Springs at a young age. Designed for pre-school & kindergarten age children, these 45 minute classes will provide a comfortable and fun group environment as participants learn the fundamentals of basketball. Each session will be overseen and run by a certified Christopher Spartz trainer and focus on learning the game, skill development, and providing a structured, engaging environment to get the children excited about the game!

    FALL BASKETBALL CLINIC
    Open to Boys & Girls
    Get in the gym, work on your skills and have some fun tuning up for your winter travel or house league teams. The camp will be run by Christopher Spartz Basketball trainers.

    BALLERZ
    The Ballerz workouts will be run by Christopher Spartz Basketball elite trainers to ensure that the experience for your developing basketball player is positive and fun. In these formative years, it is extremely imperative that your child receives the best teachers and learn the proper fundamentals in order to excel quicker. The sooner they see the gains, the more fun they have. Our points of emphasis will be: Fundamentals 101; how to have fun playing the game; how to create great habits; sportsmanship and rules of the game.

    FALL ELITE WORKOUTS (ALL 6 DAYS OR PICK 4 DAYS)

    These workout sessions were created upon request from our Blazer and Wildcat families. The growth of the players program-wide throughout this winter and spring seasons has been tremendous. We are thrilled that our families wanted more time in the gym with our CSB trainers throughout the summer and fall months. Our goal is to keep your sons’ and daughters’ skills sharp in the offseason and to do it with their teammates and players within the Blazer and Wildcat programs.

    540455 - TOTAL BODY CONDITIONING (KELLY O'DONNELL)

    This is a full body strengthening workout to include core, balance and cardio. We will incorporate hand weights, band and blocks provided by the Recreation Dept. This is an all-level fitness class. Bring your own mat and water.

    Individual classes are $10.00 ($8.00 for Seniors 65+). Codes below are for 5-class Punch Cards ($50.00 ($40.00 Seniors 65+)). Cards expire after 6 months from date of purchase. No refunds for unused classes without a doctor's note. Card is non-transferable.

    IMPORTANT NOTICE:
    Due to Covid protocols, each class is limited to 20 participants. We are also in the process of transferring over to a new computer system. To add your name to each day's attendance list, you must go to our new site at WWW.WSPRINGS.RECDESK.COM and create a new account. You must then register for each individual class you will be attending. You will still be able to use your punch card and will not be charged a fee on the new system.     Math Plays I (Parent and Tot)
    This virtual class is designed for parents/guardians and children to start using "math talk" as they participate in math concepts necessary for beginning math success. Concepts may include colors, shapes, counting, one-to-one correspondence, matching, sorting and classifying. Parents/guardians will receive emails with all of the information and videos for each class. Emails will be sent the morning of each class.

    Math Plays II (Parent and Tot)
    This virtual class is designed for parents/guardians and children to continue using "math talk" as they participate in math concepts necessary for continued math success. Concepts may include colors, shapes, counting, one-to-one correspondence, matching, sorting and classifying. Concepts will build on those practiced in Math Play I, however you do not have to take Math Play I to be eligible for II. Parents/guardians will receive emails with all of the information and videos for each class. Emails will be sent the morning of each class.

    Virtual Math Plays (Preschool-Kindergarten)
    This may be your first virtual class experience or as an extension of a current class/curriculum. Concepts may include colors, shapes, counting, one-to-one correspondence, matching, sorting and classifying, comparing, patterns, basic addition and subtraction, and data/charts/graphs. Class activities may include themed math songs, felt board activities, free play of predetermined games/activities, written enrichment, and a story and math craft. Parents/guardians will receive emails with all of the information and videos for each class. Emails will be sent the morning of each class.

    Math Plays (Homeschooled 1st-4th Grades)
    During these virtual sessions, concepts for 1st & 2nd graders may include place value, ordering, comparing, rounding, addition and subtraction, length measurement, time, and money. Concepts for 3rd & 4th graders may include place value, ordering, comparing, rounding, multi-digit addition and subtraction, multiplication and division, and area and perimeter. Class activities may include instruction on new concepts, independent play of predetermined games/activities, and written enrichment. Parents/guardians will receive emails with all of the information and videos for each class. Emails will be sent the morning of each class.

    MANNERS MATTER - AGES 7-11
    Looking to help your child gain the poise and confidence of social etiquette? Children will learn the basic skills needed to help them navigate in various social situations with ease and certainty. The focus is on teaching manners as a demonstration of kindness, consideration and respect toward others. This class offers the ‘other voice’ to convey the common courtesies that parents try to teach at home. Topics include: introductions, conversation, telephone and technology manners, writing thank-you notes, and table manners. Dress code (optional): young ladies-dress/skirt and blouse, and young men-khakis and collared shirt.

    Act and Dress for Success!! Teens will learn how to make a great first impression, give a proper introduction, properly use telephone and email etiquette, write thank-you letters, make small talk, use technology and social media (‘netiquette’) responsibly, and dress for the occasion. We will also review table manners and provide a dining tutorial. This class offers the ‘other voice’ to convey the common courtesies that parents try to teach at home.     NEW FRIDAY CLASS ADDED! 9:00-10:00 AM. BEGINS OCTOBER 16.

    This class incorporates hand weights for body sculpting to increase bone density, gain lean and defined muscles,improve balance, enhance flexibility and boost metabolism. Classes include Interval Training (HIIT and Tabatas) and Yoga Sculpt. Exercise equipment such as resistance bands, bosu balls, steppers, yoga blocks and straps will be incorporated in classes and are provided. Although this is an intermediate level class, modifications will be provided and all levels are welcome. Bring a mat, weights, and water bottle and wear gym shoes.

    Individual classes are $10.00 ($8.00 for Seniors 65+). Codes below are for 5-class Punch Cards ($50.00 ($40.00 Seniors 65+)). Cards expire after 6 months from date of purchase. No refunds for unused classes without a doctor's note. Card is non-transferable.

    Don’t wait! Get in shape! Total body work-outs are set to energizing music to shape every inch! Exercises target tummies, hips, waists, heart. Enjoy 55 minute health club workouts at a fraction of the cost. We use free weights to tone and maintain muscle. Bring your own or use provided ones. Classes are coed and open to all age groups. Exercise is the only true fountain of youth. It keeps both mind and body healthy. So now is the time to enjoy and really just do it!

    Class also meets on Saturdays, 7:45-8:45 am. Fee is $5.00 per class.

    We have added a NEW option where you can sign up monthly for every Tuesday, Thursday and Saturday.
