    PROGRAM CONSENT FORM
    By registering online, I acknowledge that I understand that my child will be participating in the Western Springs Blazers/Wildcats Traveling Basketball Program. I give permission for my child to participate as outlined below. *I also acknowledge that additional fees may be owed for my child's team at the completion of the season.

    MEDICAL INFORMATION/CONSENT
    Safety is a key concern in any athletic endeavor. Your child will be participating in many basketball practices and games during the next few months. PLEASE BE AWARE THAT THERE IS INHERENT RISK OF INJURY PLAYING BASKETBALL. If you believe this activity could endanger your child, please do not sign up or drop him/her from this program. Also, please make sure your child has had a physical checkup from his/her doctor and is physically able to participate in this strenuous activity.

    Finally, due to the nature of a traveling team, the players will be playing games in other communities. If an EMERGENCY SITUATION arises involving your child, we will take quick action. The coach will make every effort to contact a child's parents, but failing to reach you, he/she will exercise his/her best judgement including making a decision to contact 911 services for assistance. The MEDICAL INFORMATION and CONSENT you provide will be with the coach during the season. If there are any questions or concerns about these items, please contact Mike Kenny at 708-246-9070 x428.

    GENERAL ACTIVITY/COVID WAIVER
    Please read this carefully and be aware that by registering for and participating in programs or by registering your minor child/ward for participation in programs you will be waiving your rights and/or the right of your minor child/ward to all claims for injuries you or your minor child/ ward might sustain arising out of programs and you will be required to indemnify, hold harmless and defend the Village of Western Springs for any claims arising out of participation in Recreation Department programs.
    RISK OF INJURY: "As a participant in the programs, or as a parent or legal guardian of a participant under 18 years of age, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of injuries, including death, damages, or loss which I may sustain as a result of participating in any and all activities associated with Recreation Department programs."
    WAIVER OF INJURY CLAIMS: "I agree to waive and relinquish any and all claims I may have arising out of, connected with, or in any way associated with the activities of the programs."
    RELEASE FROM LIABILITY: "I do hereby fully release and discharge the Village of Western Springs and its officers, agents, and employees from any and all claims from injuries, including death, damage or loss which I or my minor child/ward may have or which may occur on account of participation in the programs."
    INDEMNIFY AND DEFENSE: "I further agree to indemnify, hold harmless and defend the Village of Western Springs and its officers, agents, and employees from any and all claims from injuries, including death, damages, and losses sustained by me or my minor child/ward and arising out of, connected with, or in any way associated with the activities of the programs. In the event of any emergency, I authorize the public entity to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed reasonable and necessary for my minor child’s immediate care and agree that I will be responsible for payment of any and all medical services rendered."
    I have read and fully understand and agree to the above stated conditions of participation.

    COVID-19 MONITORING PERMISSION
    In consideration of myself and/or my child/ward being allowed to participate in the Activity, I grant the Village of Western Springs, its officials, agents or employees full permission to monitor for symptoms of COVID-19, which may include being asked a series of questions:
    --Have you had a cough or a fever?
    --Have you been in contact with anyone who recently traveled abroad, or anyone showing symptoms for COVID-19?
    I understand and agree that if I and/or my child/ward has a temperature, shows symptoms of COVID-19 or answers any questions in the affirmative, I and/or my child/ward shall not be allowed to participate in the Activity.
    I agree that I and/or my child/ward will perform a temperature scan prior to attending any Activity and will not attend the Activity if temperature is above 100.4º F. I agree not to attend the Activity if I and/or my child/ward show any symptoms of COVID-19.

    I agree to contact the Village of Western Springs Recreation Department if I and/or my child/ward tests positive for COVID-19 and have attended the Activity. (Your personal information will not be shared).
    Extension of Participant Liability Waiver and Hold Harmless Agreement. I hereby agree and understand that the terms and provisions of the Participant Liability Waiver and Hold Harmless Agreement contained on the Registration Form is extended to include COVID-19 exposure, infection or the monitoring of COVID-19 exposure as provided above.
    I have read and fully understand and agree to the above stated conditions of participation regarding Covid-19.
    Western Springs Recreation Dept and Christopher Spartz Basketball:
    Blazers and Wildcats Travel Basketball Bubble

    The WINTER program will be broken down into 2 Seasons.

    SEASON 1: November 1-January 1
    2 practices per week (14 total): all held in our Bubble at WS Rec Center; 1hr 15min practices: 2/3 CSB Skills Development Coach Run; 1/3 Coach Run

    12 potential inter squad game dates on Saturdays and Sundays: all games held in our Bubble at WS Rec Center; all games coached by coaches.

    Each coach will advise when their particular teams have been set.

    SEASON 2:* January 1-last week of February. Season 2 will be predicated on protocol updates from our State and Governor.
    *Players must commit to the first season to be a part of the second season.

    With the objectives listed above, the Rec Dept and CSB Skill Development Coaches will work hand in hand with each coach to roll out a plan of attack that can be carried out and executed through January 1. Season 1 will give the CSB Skill Development Coaches a real chance to target areas that each team needs to grow and chart their growth.

    Based on the rules and regulations handed down by the Governor, we will only be allowed to compete in inter squad games, which means we have to align our practice times with the teams they will be scrimmaging against. We will know what that looks like when we have our teams in place.

    SENIOR CENTER MEMBERSHIP
    Membership, which is open to the general public, provides participation in our community of activities and events geared specifically to the interests of the older adult. Membership also supports the need of seniors to socialize and make new friends. We are located at the Grand Avenue Community Center, 4211 Grand Avenue, Western Springs, 2nd floor in the Neal Room (elevator access available).     GOLD level membership is $50/Residents, $60/Nonresidents and includes all activities including unlimited exercise and unlimited classes.

    SILVER level membership is $40/Residents, $50/Nonresidents and includes all activities except exercise and classes.

    BRONZE level membership is $20/Residents, $30/Nonresidents and includes library book borrowing, movie borrowing, movie watching, and regular bridge subs.

    Spouses are ½ price for each level.

    YOGA FOR YOUNGSTERS
    During this class we will explore yoga by learning basic poses. Stories and games will be included in every class to enhance the experience. Children will learn calming techniques, increase their strength and flexibility in addition to a variety of yoga poses. Join this exciting class!!

    YOGA AND SUPER HERO WORKSHOP
    This exciting one day workshop is a combination of Yoga and Superhero-themed activities. In the first hour, participants will enjoy superhero activities including wearing superhero party masks and temporary tattoos to dress up as a superhero, choosing their own superpowers and making superhero crafts. Afterwards, participants will enjoy a second hour of Yoga instruction where they will enhance their flexibility and strength and learn a variety of yoga poses, or mudras, while playing fun yoga games. Yoga mats will be provided. Participants should bring a water bottle. Younger children must be fully potty trained.

    Illinois Shotokan Karate Club is Chicagoland’s premier karate school taught under the instruction of John DiPasquale, a four-time National Champion, U.S. National Coach and President of the U.S. National Karate Federation.

    PRE-KARATE (4-7 years)
    Young children develop flexibility, strength, and coordination in fun, yet disciplined activities. This program is specifically designed for growing minds and bodies to enhance memory and agility. Watch your child’s confidence blossom and their skills improve.

    PARENT/CHILD (7 years and up)
    Parent/Child classes are a great way to spend quality time with your child, while conditioning muscles, developing coordination and improving cardiovascular fitness. Both parent and child must each sign up for class separately.

    YOUTH/ADULT (8 years and up)
    These classes will put students in touch with their bodies and offers a good mix of strength training, with cardiovascular and flexibility exercises. Students will increase their coordination, agility and poise, as well as learn lasting personal safety skills. They also benefit from the mental stimulation of learning the choreographed moves of the kata and the discipline of conforming to the class rules.

    - Uniforms and Belt-Testing are options available through the ISKC instructor.
    - Please note that the Youth/Adult and Parent/Child classes may train together.
    - Beginner students will not be permitted to register into the current session after the second class has met. Registration for continuers through advanced students will be accepted after the start date, but the full class fee is required.

    Tot Rock
    Wiggle and giggle to the Tot Rock beat! An introductory music and movement class with a large sensory component, tots will use hand-held rhythm instruments and play props to improve their motor skills and coordination, and learn new songs and rhymes. While kick starting your tot’s imagination, each class will provide structured and unstructured time to explore. A different educational theme presented each session. Much more than a music class, join us to sing, dance, learn and play the Tot Rock way!

    Kid Rock III
    Music fun that can’t be "beat!" Each class includes creative, energetic activities based on a new educational theme each session! Children learn new songs, rhymes, and gross motor skills while using rhythm instruments and movement props. Your child’s listening skills will develop, while fine motor and coordination improve through imaginative play and sensory integration. Much more than a music class, join us to sing, dance, learn and play the Kid Rock way!

    VIRTUAL OPTION FOR BOTH TOT ROCK AND KID ROCK!!
    Rock 'n' Kids is Rockin' at Home! Enroll in the virtual class session to sing, dance, play and learn! Registered participants will receive an email from Rock ‘n’ Kids with that week's pre-recorded, private, 30-minute virtual class, allowing families to participate on their own time. The session's musical activities will be based on the theme of "A Whole New World!" Each week we will pretend to ride on our magic carpets to explore a new place! A pirate ship? The jungle? Outer space? It will be a musical adventure!

    520245 - LACROSSE-BOYS HOUSE-FALL

    A NOTE TO ALL OF OUR PARENTS AND PLAYERS PARTICIPATING IN OUR LACROSSE PROGRAM: With the unknown of how the guidelines will change for youth sports in the next few weeks, these are the programs we are currently offering. We will be following all local and state guidelines strictly, and strive to keep the health and safety of our participants, their families and our staff at the forefront of all we do. All of our practices and training will be non-contact.

    INTRODUCTION TO LACROSSE-BOYS AND GIRLS - 2 SESSIONS OFFERED: 9/13-10/18 and 11/1-12/13.
    Kindergarten-2nd Grade
    Introduce your child to the exciting sport of lacrosse. Our spring league coaches will instruct your child on the basics of lacrosse. If your child is under the age of 6, a parent must stay on the field with their child.
    REQUIRED EQUIPMENT: We will use a soft ball. Bring your own stick (STX Boys' Stallion 50 Lacrosse Stick at Dick's Sporting Goods. Use Dick's Everyday Coupon for 10% off (located on our website at www.wsprings.com/lacrosse)). or we have sticks available. All other equipment is optional.

    BOYS HOUSE LEAGUE
    Beginners and experienced (3rd - 8th grade)
    SESSION 1 - Code 520245 (September 13 - October 18)
    SESSION 2 - Code 620245 (November 1 - December 13 (No class 11/27))
    Focus will be on learning lacrosse skills in offense, defense and game strategies. Emphasis in scrimmages will be on skill development, teamwork, and sportsmanship.
    REQUIRED EQUIPMENT: Lacrosse stick, mouthpiece, lacrosse or hockey helmet with face guard; shoulder pads, elbow pads, lacrosse gloves, and cup.

    LIL HOOPERS
    Let the first time your son or daughter pick up a basketball be with Christopher Spartz Basketball’s Lil Hoopers! This is a great way to get acclimated with the premier basketball program of Western Springs at a young age. Designed for pre-school & kindergarten age children, these 45 minute classes will provide a comfortable and fun group environment as participants learn the fundamentals of basketball. Each session will be overseen and run by a certified Christopher Spartz trainer and focus on learning the game, skill development, and providing a structured, engaging environment to get the children excited about the game!

    FALL BASKETBALL CLINIC
    Open to Boys & Girls
    Get in the gym, work on your skills and have some fun tuning up for your winter travel or house league teams. The camp will be run by Christopher Spartz Basketball trainers.

    BALLERZ
    The Ballerz workouts will be run by Christopher Spartz Basketball elite trainers to ensure that the experience for your developing basketball player is positive and fun. In these formative years, it is extremely imperative that your child receives the best teachers and learn the proper fundamentals in order to excel quicker. The sooner they see the gains, the more fun they have. Our points of emphasis will be: Fundamentals 101; how to have fun playing the game; how to create great habits; sportsmanship and rules of the game.

    FALL ELITE WORKOUTS (ALL 6 DAYS OR PICK 4 DAYS)
    Open to Boys & Girls
    These workout sessions were created upon request from our Blazer and Wildcat families. The growth of the players program-wide throughout this winter and spring seasons has been tremendous. We are thrilled that our families wanted more time in the gym with our CSB trainers throughout the summer and fall months. Our goal is to keep your sons’ and daughters’ skills sharp in the offseason and to do it with their teammates and players within the Blazer and Wildcat programs.

    YOGA AND SLIME ECLIPSE WORKSHOP
    This one day workshop incorporates both yoga as well as a slime making workshop! The yoga session is an introductory course for beginners where students will learn basic poses and calming techniques while increasing their strength and flexibility. Stories and games will be included to enhance the experience. Yoga mats will be provided. Participants should bring a water bottle. Note: to make the slime, we may be using school or PVA glue, baby power, acrylic paint, food coloring, lotion, shaving cream, as well as laundry detergent. Class requires 3 business days notification to cancel.

    520668 - FENCING

    Want to wield a sword? Learn the basics of attack and defense as well as strategies that create the foundation for fencing. Each class will have instruction and bouting (actual fencing). All equipment provided. This class is for beginners and advanced fencers and is family friendly. Fencing is statistically the most and least injurious sport as reported by the US Olympic Committee! Sign up for this surprisingly fun and memorable sport!

    Due to Covid-19, fencers must rent equipment for the session or purchase their own basic set (est. $140). To rent a personal set of equipment, contact Fencing Sports Club at 630-678-0035. If you prefer to purchase a personal beginner set, the Club is happy to quickly assist with ordering from Blue Gauntlet. Fencer must attend first class with their new set in hand. At the end of session, you may have the Club buy your equipment at 50% of purchase price if no longer interested in owning.

    A NOTE TO ALL OF OUR PARENTS AND PLAYERS PARTICIPATING IN OUR LACROSSE PROGRAM: With the unknown of how the guidelines will change for youth sports in the next few weeks, these are the programs we are currently offering. We will be following all local and state guidelines strictly, and strive to keep the health and safety of our participants, their families and our staff at the forefront of all we do. All of our practices and training will be non-contact.

    Western Springs Lacrosse offers seasonal options for Boys Travel: Fall (codes 530245) and Winter (codes 630245).

    The program's goal is to prepare all our players all year long by offering more advanced skills and development opportunities. Our experienced and gifted coaches will provide the tools and knowledge to develop each player to the highest level. Our philosophy is that positive motivation and encouragement will elevate players understanding of the game to a level appropriate for competitive games.

    Base Fee includes coaches, fields, team supplies, and administrative cost. There will be an additional fee for tournaments estimated at $200. Tournaments to be determined. All dates, fees and information for lacrosse are subject to change.
